Why Retatrutide Isn't Just "Stronger Semaglutide"

Semaglutide hits one receptor. Retatrutide hits three. That's a difference in kind, not just in dose — and it explains the slower titration curve.

Semaglutide works by agonizing a single receptor: GLP-1. It's an effective and well-studied mechanism — GLP-1 receptor activation slows gastric emptying, increases satiety signaling, and improves insulin secretion in response to meals. Retatrutide is often talked about as if it's just a more potent version of the same thing. Mechanistically, it isn't: it's a triple agonist, activating GLP-1, GIP, and glucagon receptors simultaneously.

The GIP receptor adds a second incretin pathway that works alongside GLP-1 rather than duplicating it, and in combination-agonist research is associated with improved insulin sensitivity beyond what GLP-1 activity alone produces. The glucagon receptor component is the more unusual addition — glucagon classically raises blood sugar, which sounds counterproductive in a metabolic drug. But at the receptor level, glucagon agonism increases energy expenditure and hepatic fat oxidation; combined with the insulin-sensitizing effect of the other two pathways, the net effect in the research is improved metabolic outcomes rather than the hyperglycemia glucagon alone would suggest.

This is also the direct explanation for why triple agonists get titrated so much more slowly than single-receptor GLP-1 drugs. Each additional receptor pathway is an additional source of GI-tract signaling — slowed gastric emptying from GLP-1, plus the GIP and glucagon effects layered on top — and GI tolerability, not a hard dose ceiling, is what paces how fast the dose can be raised. A protocol that jumps to an "effective" dose in week one isn't being aggressive, it's skipping the step that determines whether the compound is tolerable at all.

The practical upshot: more receptors means more mechanism, but it also means more places for side effects to originate, and a titration schedule is doing real mechanistic work, not just being cautious for its own sake.
